This notice describes information submitted through JFZ Final Expense coverage inquiries and agent applications, plus public-site tracking and staff access.
Information we collect
We may collect information you choose to provide, such as your name, age, state, phone number, email address, household and coverage goals, existing insurance information, tobacco-use response, desired coverage, budget, preferred contact time, and the pineapple-on-pizza rapport answer.
If you submit an agent application, we may also collect your insurance-license status, sales experience, schedule preferences, recruiting message, and related application details.
We may also collect the page URL, referring page, submission time, campaign parameters, and advertising click identifiers such as UTM values, GCLID, FBCLID, or MSCLKID when they are present in the page URL.
Please do not submit Social Security numbers, banking information, payment details, policy numbers, medical records, passwords, or other information this website does not request.
How we use information
We use submitted information to respond to your inquiry, understand your requested coverage needs, communicate with you, review recruiting interest, maintain records of your request and consent, prevent fraud or abuse, measure advertising performance, and comply with applicable legal or regulatory obligations.
How information may be shared
The website is hosted on Netlify. Its forms and CRM use Netlify services to receive and store requests and control staff access. The configured email integration uses Resend when enabled. The staff calling integration uses Twilio when configured. These service providers process information needed to perform their functions; configuration and actual use should be reviewed with JFZ.
Information may be shared as reasonably necessary with licensed insurance representatives, insurance carriers when appropriate for an insurance inquiry or application, website and communications service providers, professional advisers, or regulators and law-enforcement authorities when required by law. Submitting this form is not an insurance application.
Retention and security
We use reasonable administrative and technical safeguards appropriate to the information collected. No internet transmission or storage system can be guaranteed completely secure. Information is retained only as reasonably necessary for the purposes described above, recordkeeping, dispute resolution, fraud prevention, and legal obligations.
Your choices
You may decline to submit the form. You may also request that future communications stop using the opt-out method provided in the communication or by telling the representative who contacts you.
Optional analytics and advertising measurement
With your permission, Google Tag Manager may load analytics or advertising measurement tools to understand website visits and successful inquiries. These tools may use cookies and similar technologies. You may reject optional tracking or change your choice below. Rejecting optional tracking does not prevent you from submitting a coverage inquiry.
The website sends generic form-start, step-completion, and successful-inquiry events for measurement. We do not intentionally send your name, contact details, tobacco response, coverage details, or questionnaire answers as analytics event parameters. Any additional analytics or advertising tags should be configured consistently with these restrictions.
Privacy requests
Email support@jfzfinalexpense.net to ask about access, correction, deletion, or contact restrictions. JFZ may need to verify your identity and retain records needed for legal, insurance, security, or suppression purposes. Rights and exemptions depend on your jurisdiction and the information involved. Do not email identity documents unless a secure process has been arranged.
Recruiting applications and consumer inquiries are kept in separate workflows. Pipeline movement does not remove recorded opt-outs. Read the Cookie & Tracking Policy for browser choices.
Changes to this notice
This notice may be updated as the website, service providers, or legal requirements change. The effective date above identifies the current version.
